-You attest that you have been made aware of the policy’s details, main benefits , exclusions and key features.
-You attest and understand that the long form policy information will be sent to you directly from the insurance carrier, along with your ID cards.
-You attest that you will have full access to your policy information.
-You attest that it is your responsibility to understand, what is covered in your policy, and what is not covered in your policy
-Finally, you attest that you can contact the insurance carrier directly for any help on policy information or with help submitting a claim.
